DANCE FOR SICK KIDS FUNDRAISING EVENT – WAIVER, INDEMNITY AND RELEASE FORM 

I wish to participate in Dance for Sick Kids to raise funds for Ronald McDonald House Charities Greater Western Sydney (RMHC GWS). 

Dance for Sick Kids is a fundraising campaign scheduled through the month of May 2020 and involves me fundraising in a manner of my choosing for RMHC GWS (Event). 

This agreement applies to the Event and any other related event(s) in which I engage to raise funds for RMHC GWS. 

I assert and confirm that my participation in the Event is voluntary. 

I understand that if I am under the age of 18, I MUST have a guardian approve my participation in the event. 

I acknowledge that the Event is being run to fundraise for RMHC GWS and I agree to abide by all rules, regulations, and event instructions of the Event, as well as all applicable municipal and state laws and regulations. 

I am aware of and expressly assume all risks associated with participating in this Event. 

I understand and acknowledge that RMHC GWS is not in a position to judge my capacity, fitness or any other matter in relation to my ability to participate in the Event. I assure RMHC GWS that it has no reason to conduct any special inquiry into my capacity, fitness, or any other matter relating to my ability to participate in the Event and I am not aware of any illness, injury or any other matter which may cause me injury while participating in the Event. I request RMHC GWS to treat my request for authority to fundraise as being from a person fully capable of preparing, organising, holding and participating in the Event. 

I agree to put in place appropriate precautions before participating in the Event to ensure that any necessary medical assistance will be available to me during and after the Event. I acknowledge that RMHC GWS is not responsible for organising or paying any costs involved with such medical assistance. 

I agree to release, indemnify and hold harmless RMHC GWS, any beneficiaries, sponsors, officials, participating clubs, communities, organisations, friends of the 

Event, volunteers, consultants, participants, third-party vendors, government or public entities) and each of their respective affiliates, successors, officers, directors, employees, volunteers, agents, and representatives, including, without limitation, Event medical sponsors, medical director, and members of the medical team from all claims, actions, demands, suits or proceedings of any nature whatsoever which may be made by me, on my behalf, or by or on behalf of any other person, in respect of or arising out of any liability, loss, cost, damage or expense, including but not limited to damage to property, injury or death, that I or any other person suffer during the course of, as a result of, or that is associated with, the Event, (including during any preparation or planning for the Event or any associated activity), and however caused (whether due to the negligence or omission of RMHC GWS, its officials, employees, agents, contractors, volunteers or other personnel or otherwise). 

I acknowledge that I am not relying, and will not rely, on RMHC GWS or any representation made by RMHC GWS or its officials, employees, agents, contractors, volunteers or other personnel in any way, including in preparing for, organising or participating in the Event. 

I agree that my participation in the Event is subject to the sole discretion of the organisers of the Event, and that my participation may be limited or terminated, with or without cause. 

I acknowledge that without authority from RMHC GWS to fundraise, I am not permitted to identify RMHC GWS as a charity associated with the Event, or to use the RMHC GWS name or logo or any other material associated with RMHC GWS in preparing for, organising or participating in the Event, including but not limited to marketing or promoting the Event. 

I acknowledge and agree that RMHC GWS may, in its absolute discretion, withdraw my authority to fundraise that RMHC GWS grants me, upon which I must immediately cease the Event and discontinue any use of the RMHC GWS name and logo. 

I agree that any photographs, video, film footage or audio recording of me taken as part of the Event may be used by RMHC GWS, Dance for Sick Kids and each of their respective affiliates, subsidiaries and agents for the purposes of promoting Dance for Sick Kids and RMHC GWS and I hereby perpetually and irrevocably consent to any such use without payment or compensation and waive any rights I might have to object to such uses under parts IX and XIA of the Copyright Act 1968 (Cth). 

I understand and agree that these terms are binding on my heirs, assigns, and legal representatives. 

In return for RMHC GWS considering my application for authority to fundraise, and, should RMHC GWS grant me authority to fundraise, in return for that grant, I agree to the above terms. 

This agreement is governed by the laws of the State of New South Wales and the New South Wales courts shall have exclusive jurisdiction for any dispute arising under or relating to this agreement. 

I have carefully read this form, understand its contents and agree that its terms and conditions bind me and my heirs and successors. 


DFSK SOCK FLASH CHALLENGE

All DFSK participants that receive two donations within the nominated time frame will receive a pair or DFSK socks. Adults or Children's size available dependent on over/under 18 years selected on registration. One prize per person. The prize winner must update postal address withing fundraising dashboard for prize to be posted out. Ronald McDonald House Charities GWS takes no responsibility for postal delays or lost post. We do not gurantee winner will receive prize within any set time frame. 

RAGING WATERS SYDNEY DOUBLE PASS FLASH CHALLENGE


75 double passes available for the first 75 Dance for Sick Kids participants to receive a minimum of two donations within the agreed time frame (18 May 3:00pm AEST - 19 May 3:00pm AEST). One prize per person. Each prize includes 2x Day passes to Raging Waters Sydney (total value $150, valid for 2020/2021 season only, non transferable or redeemable for cash).

DOLLAR MATCHING

Please be aware that our dollar-matching  pool is not limitless. Donations will be matched on a first come, first served basis up to the value of  $10,000. Matched donations will run from 6:30pm AEST Wednesday 20 May until 6:30pm AEST Thursday 20 May or until otherwise exhausted. Only sponsored donations made to Ronald McDonald House Charities Greater Western Sydney will be matched.

PLEASE NOTE: Donations made from within your fundraising dashboard will not be doubled.

We will email all participants as soon as the dollar-matching pool has run out and post in our DFSK Facebook Group.  

DFSK COMPETITION #DFSKCOMP

The promoter is Ronald McDonald House Charities Greater Western Sydney Ltd (ABN 22 003 000 806) whose registered office is at 1 Labyrinth Way Westmead NSW 2145

  1. The competition is open to registered participants of Dance for Sick Kids.
  2. There is no entry fee to enter the competition however you must be a registered participant of Dance for Sick Kids.
  3. By entering this competition, an entrant is indicating his/her agreement to be bound by these terms and conditions.
  4. Route to entry for the competition and details of how to enter are can be found at www.danceforsickkids.com.
  5. Video entries will be submitted via Instagram and/or TikTok by using #danceforsickkids & #DFSKComp. Alternatively, they can be emailed to supportinggws@rmhc.org.au. Your fundraising page will be judged directly from the website.
  6. Closing date for entry will be Midnight Sunday 24th May 2020. After this date the no further entries to the competition will be permitted.
  7. No responsibility can be accepted for entries not received for whatever reason.
  8. The promoter reserves the right to cancel or amend the competition and these terms and conditions without notice. Any changes to the competition will be notified to entrants as soon as
    possible by the promoter.
  9. The promoter is not responsible for inaccurate prize details supplied to any entrant by any third party connected with this competition.
  10. The prize is as follows:
    1 x 1hour 30minute virtual zoom meeting with Marko Panzic for the chosen 3 teams and chosen 3 individuals. 1 hour of dance lesson and 30minutes of Q&A’s.
    1 x 1hour 30minute virtual zoom meeting with Jarryd Byrne for the chosen 3 teams and chosen 3 individuals. 1 hour of dance lesson and 30minutes of Q&A’s.
    The prize is as stated and no cash or other alternatives will be offered. Time and data of zoom meetings will be determined once winners are chosen. 
  11. Winners will be chosen:
    Winners will be chosen based on a combination of their fundraising and dancing.
    Judges will be Marko Panzic, Jarryd Byrne & Belinda Woolford.
  12. The winner will be notified by email and phone within 28 days of the closing date.
    If the winner cannot be contacted or do not claim the prize within 14 days of notification, we reserve the right to withdraw the prize from the winner and pick a replacement winner.
  13. The promoter will notify the winner when and where the prize will take place.
  14. The promoter’s decision in respect of all matters to do with the competition will be final and no correspondence will be entered into.
  15. By entering this competition, an entrant is indicating his/her agreement to be bound by these terms and conditions.
  16. The winner agrees to the use of his/her name and image in any publicity material, as well as their entry.
  17. Entry into the competition will be deemed as acceptance of these terms and conditions.

Additional Terms and Conditions:

Marko Panzic, Jarryd Byrne & Belinda Woolford will together judge the competition and decide on the winner within 28 days of the final submission date.

Ronald McDonald House Charities Greater Western Sydney also reserves the right to cancel the competition if circumstances arise outside of its control.

Dance for Sick Kids Survey Competition - BLOCH merchandise packs

This is a competition run and operated by Ronald McDonald House Charities Greater Western Sydney (RMHC GWS).  
The prize is 5 BLOCH merchandise packs that 5 individuals will win. To be eligible to win, you must complete the Dance for Sick Kids survey which will be emailed to all participants that have given a valid email address upon their registration. Each pack is valued at over $100 RRP (non-transferable or redeemable for cash). 
The winners will be drawn by a random result generator. Submission of the survey must be complete by 1st July 2020 to go into the draw. The draw will take place within 7 days of this date and winners will be notified via email within a further 7 days post draw.  
RMHC GWS accepts no responsibly for prizes lost in the post or prizes affected by postage delays. In the event a prize is lost or stolen, RMHC GWS is unable to replace said prize. RMHC GWS reserves the right to cancel the competition if circumstances arise outside of it’s control.
